Capital Health offers a program that supports congestive heart failure patients at both hospitals by providing them with optimized care while they are admitted—including education about their unique needs and providing a post-discharge plan for when they return home. Based on medical best practices outlined by the American Heart Association’s Get With the Guidelines®-Heart Failure program, this program is meant to provide patients shorter lengths of stay, fewer trips to the ER, and the educational tools they need to make healthy choices after their stay with us.
