To ensure all credentials are received, please send them in one packet by the March 31 deadline for the LPN to RN program or the April 30 deadline for the generic program. A completed application includes the following:
- Applicant Submission Form
- Application for Admission
- $50.00 nonrefundable application processing fee, payable by cashier’s check, certified check, or money order only, made payable to Capital Health School of Nursing – Regional Medical Center.
- One (1) official high school transcript.
- One (1) official transcripts from ALL other educational institutions you have attended since high school. Official transcripts must be in separate sealed envelope with an official signature or stamp across the seal.
- Three (3) professional references (teacher/instructor, employer/supervisor, community service representative/clergy).
- Official Entrance Exam results (TEAS all applicants; TOEFL International applicants). If proctored by Capital Health you will not need to include the results.
