Getting Started with Diabetes Management

Mondays, 3 - 4 p.m.
Location: Online via Zoom or in person at Capital Health – Hamilton

Are you newly diagnosed with diabetes? Learning how to use your glucose meter for the first time? Just starting insulin? Spend an hour with our diabetes education team to learn the basics of your diabetes care and have your questions answered. Topics to be discussed will vary with each class according to questions and needs of those present.

Speak with the Certified Diabetes Care & Education Specialist

Mondays, 3 - 4 p.m.
Location: Online via Zoom or in person at Capital Health – Hamilton

Are you newly diagnosed with diabetes? Have you had diabetes for years but still feel out of control? Do you have questions regarding your management of your diabetes? A certified diabetes care and education specialist (CDCES) is a healthcare professional who possesses comprehensive knowledge and experience in the management of diabetes, pre-diabetes, and diabetes prevention. Spend an hour with our diabetes education team to get answers to any questions or concerns you have related to your diabetes care.

Topics to be discussed will vary with each class according to questions and needs of those present. Topics may include:

  • Managing Pre-Diabetes
  • Knowing Your Numbers – A1C, blood glucose goals, carb count, weight, blood pressure, and cholesterol
  • Foot and Skin Care
  • Preventing Complications
  • Medications
  • Exercise Principles

Speak with the Dietitian: Meal Planning for Diabetes

Thursdays, 3 - 4 p.m.
Location: Online via Zoom

Were you recently diagnosed with diabetes or pre-diabetes? Have you had diabetes for a long time and are still unsure about what to eat? Are you confused by carb counting and food labels? This class will focus on how foods affect your blood glucose. We will discuss meal planning strategies such as the plate method, carbohydrate consistency, meal timing, portion sizes, and snacks.

The class is led by Mindy Komosinsky, our registered dietitian/certified diabetes care and education specialist. Bring your questions and learn what you need to know to help manage your diabetes.

Diabetes Prevention Class

Wednesdays, 12 - 1 p.m.
Location: Online via Zoom or in person at Capital Health Medical Center – Hopewell

Have you been diagnosed with pre-diabetes? Do you have family history of diabetes? Are you concerned about developing diabetes in the future? A certified diabetes care and education specialist (CDCES) is a healthcare professional who possesses comprehensive knowledge and experience in the management of diabetes, pre-diabetes, and diabetes prevention. Spend an hour with our diabetes education team to learn how you can prevent diabetes through modest lifestyle changes.

Diabetes Support Group

Wednesdays, 6 - 7 p.m.
Location: Online via Zoom

Capital Health is pleased to launch a new support group for anyone with diabetes or caring for someone with diabetes. Led by a member of our diabetes education team, this time together will focus on connection and education.

Have you recently been diagnosed with diabetes or pre-diabetes? Have you had diabetes for a long time and feel overwhelmed or confused? Do you feel alone or isolated in your journey with diabetes? Please join us for this free support group!

Diabetes Education Daytime Series

Mondays, 9 a.m. - 2:30 p.m.
Location: Capital Health – Hamilton or Capital Health Medical Center – Hopewell

Capital Health offers a comprehensive outpatient diabetes education program that empowers patients to take control in the successful management of their disease. A physician referral is required for participation. This program is recognized by the American Diabetes Association and includes:

  • Group sessions taught by a multidisciplinary staff
  • 8-week individual follow-up with a certified diabetes care and education specialist
  • Individual appointments are available based upon your physician’s recommendation

Tuesdays, 6 - 8:30 p.m.
Location: Capital Health – Hamilton or Capital Health Medical Center – Hopewell
