Part of our referral-based, comprehensive outpatient Diabetes Self-Management Education and Training Program, this four-week series of 90-minute group classes empowers individuals to take control in the successful management of their diabetes. Learn from a certified diabetes care and education specialist, nurses, dietitians, and other health care providers to gain the knowledge and skills to manage your diabetes with confidence.
Class 1: Diabetes management, pathophysiology, chronic complications, medications (oral and GLP-1).
Class 2: Healthy eating (part 1), lifestyle and healthy coping.
Class 3: Monitoring, acute complications, medication (insulin), distress and support.
Class 4: Being active, healthy eating (part 2).
A referral for diabetes education from your physician is required to participate in these classes. Daytime classes are held Wednesdays from 9:30 – 11 a.m. at Capital Health – Hamilton. Evening classes are held Thursdays from 5:30 – 7 p.m. at Capital Health Medical Center – Hopewell.
