What to bring

To help expedite the registration process, please provide the following information that applies to your service.

  • Identification: This will insure that the spelling of your name, social security number, and date of birth are correct.  Examples of acceptable forms of ID include a valid drivers license, a valid photo ID, and/or a Social Security Card. Please note: an insurance card is not a legal form of ID.
  • Insurance Information: Health insurance cards, and insurance billing address. Please note: when your insurance company requires it, please also bring pre-authorization form, referral form or both.
  • Physician referral: A prescription or an insurance referral form that states the service your physician is requesting. If your insurance requires a referral and you do not obtain the referral, the cost of your services may not be covered by your insurance company.
  • Self-Referral: If your insurance allows, you may refer yourself for an evaluation by our sleep physicians.
