Our Services

Capital Health LIFE is an all-inclusive care provider offering a wide range of essential services that promote independence and keep participants connected to their community. Our multidisciplinary team will ensure each participant’s needs are met through services that include:

Medical Care

The dedicated physicians, nurse practitioners, and specialists at our center in Bordentown, New Jersey see seniors for sick visits as well as routine appointments and monitor changes in their health while they enjoy other services. Medical services include:

  • Primary care
  • Nursing care
  • Specialty care
  • Medications
  • Vision and hearing care
  • Dental care
  • Foot care
  • Mental health services
  • Personal response systems
  • Medical equipment and supplies
  • Hospitalizations and nursing home care (if either become necessary)

Rehabilitation Therapy (includes physical, occupational, and speech therapies)

Skilled therapists specializing in senior care help Capital Health LIFE participants determine which therapy is best for each individual’s needs. In close coordination with care teams, our therapists offer physical, occupational and speech therapies with techniques and equipment designed to regain and improve functionality and support more independent living.

Transportation

To assist seniors who face special challenges in getting to and from medical appointments, we offer free transportation to and from our Capital Health LIFE center and medical appointments. Our trained drivers operate wheelchair-friendly vans to provide safe and comfortable access to care.

Recreational Therapy

Staying active can help seniors keep their minds and bodies sharp, and Capital Health LIFE offers opportunities for fun and fitness. In addition to improving physical and cognitive abilities, recreational therapy also creates a positive social environment. Certified recreational therapists organize a range of engaging activities for participants, including:

  • Chair yoga or aerobics
  • Bingo and trivia
  • Arts and crafts
  • Day trips
  • Computer Classes
  • Holiday and cultural celebrations

Social Services

Capital Health LIFE wants seniors to be connected to the community resources they need to make informed decisions. Our compassionate social workers serve as liaisons between participants and their care team, supporting seniors through advocacy, counseling and caregiver education, and access to resources such as financial or legal guidance.

Nutrition Planning

Dietary needs are a critical part of anyone’s care plan. With a focus on nourishment and wellness, Capital Health LIFE’s registered dietitians create meal plans tailored to each participant’s specific nutritional needs by evaluating current health challenges, risk for disease, and other factors. Capital Health LIFE provides a nutritious breakfast and lunch at the LIFE center and home-delivered meals can be arranged, as necessary.

In-Home Support

To help seniors accomplish daily tasks and live at home for as long as possible, in-home support is available, if necessary. Personal care aides come to the participant’s home to assist with routine tasks such as bathing and dressing, light meal preparation, laundry and light housekeeping.

Specialty Services

When necessary, specialty medical services such as dental, podiatry, optometry, and audiology care and supplies are available from our network of medical specialists. Referrals for specialty care must be approved by Capital Health LIFE’s medical director and a participant’s multidisciplinary care team.

 

There are no out-of-pocket charges for participants who qualify for both Medicare and Medicaid (or Medicaid only). There is a monthly fee for those who have Medicare but don’t qualify for Medicaid. For participants who do not qualify for Medicaid or Medicare, there is a monthly fee. PACE participants must receive all medical care, including the services of primary care physicians and specialists (other than emergency services), through the Capital Health LIFE team and network of participating providers. Participants may be fully responsible for the cost of unauthorized or non-program services.
