Capital Health has launched a new program that brings hospital-level care to patients in the comfort of their homes. Aimed at managing capacity at Capital Health hospitals by allowing appropriate patients to receive care at their homes, Capital Health’s new Hospital at Home Program delivers personalized care through daily virtual visits with a doctor, in-person nurse check-ins and services like physical therapy, lab work or nutrition support, all arranged by a multidisciplinary team. Participants must be New Jersey residents ages 18 years or older, reside within a 30-minute drive to Capital Health Medical Center – Hopewell, and meet other clinical and safety standards for program eligibility.

Eligible patients for the Capital Health Hospital at Home Program are typically those who have conditions such as pneumonia, chronic obstructive pulmonary disease, congestive heart failure, cellulitis, and other acute but stable diagnoses that can be safely managed at home. After getting a diagnosis and a first round of treatment when admitted to Capital Health Medical Center – Hopewell, patients who are interested in Hospital at Home are assessed by a clinical review team to make sure they meet program requirements. Appropriate candidates then go home to receive personalized care that is safely coordinated by a multidisciplinary team.
Capital Health Hospital at Home patients receive daily video visits with a Capital Health Medical Group physician and in-person visits twice a day with a registered nurse. The program’s expert care team also includes pharmacists and care managers, therapists, and nutrition and social services.
Patients are kept safe and connected to their care team with around-the-clock monitoring of their vitals and all the tools they need to manage their condition in the familiar and more restful surroundings of their homes. Advanced at-home equipment includes a tablet for video visits, direct-dial phone, blood pressure cuff, scale, oxygen monitor, and blood glucose monitoring equipment. Additional equipment is also provided as appropriate for each participant’s specific condition. Capital Health’s Hospital at Home team sets everything up and provides detailed instructions for use so patients feel confident from day one.
Hospital at Home is supported by a 24-hour Command Center staffed by registered nurses to address patient concerns or emergencies. In addition to social work and case management support, services may also include wound care and dressing changes; oxygen therapy and respiratory treatments; lab draws, intravenous infusions, and medication delivery; transportation for diagnostic testing and rapid hospital transfer (when necessary); and daily meals (if desired). Upon discharge, patients receive instructions for post-program care and any follow-up medication that is needed.
