The new website features written and video profiles of the residents it serves and the caregivers who serve them. It celebrates the partnerships it has forged with government agencies, social service organizations and other community groups to create new educational pathways for city residents and to engage them in creative ways. The website explains how the THT is working to achieve its goal of expanding access to high-quality, coordinated care and to make Trenton the healthiest city in the state.

“The new website highlights the goals of the Trenton Health Team, which include expanding access to primary care; coordinating clinical care community wide; engaging residents; exchanging of information between organizations and becoming a Medicaid Accountable Care Organization,” said Dr. Ruth Perry, THT executive director.

The Trenton Health Team website provides information for residents seeking care ranging from clinic and hospital locations to services available via the internet and phone. It offers demographics that illuminate Trenton’s circumstances in caring for a vulnerable population with significant socioeconomic and health challenges, environmental conditions, and a broad range of factors that impact on a person’s health and the population’s health as a whole. THT is looking at all factors in developing and delivering care.
