To participate in our Hospital at Home program, you must be 18 years of age or older, a New Jersey resident, and live within a 30-minute drive to Capital Health Medical Center – Hopewell. Participants must require an inpatient level of care that is verified by our Utilization Review team and meet standards to receive home care (or be ambulatory, if living alone).
Hospital at Home is NOT AVAILABLE for patients with:
- Hemodynamic instability (unstable blood blow in the body).
- Altered mental status or active delirium.
- Recent substance abuse.
- Need for advanced interventions (i.e., surgery, CT scan, MRI, blood transfusion).
- Lack of essential home utilities (no running water, no functional heat/air conditioning).
- No reliable care giver support (as needed).
- Currently in police custody.
