Clinical Practicum Group Request for Placement

Directions:

1. Complete Request for practicum below

2. Email Student names and vehicle information, student health attestation forms, and signed mandatory book acknowledgements to [email protected] at least 2 weeks prior to practicum start date.

3. Email course syllabus, clinical objectives, and/or practicum requirements.

Clinical rotation will not be permitted to start until all required information and documents are received by the Department of Clinical Education.
