Patients must always bring medications, immunization records, and past medical history. Every effort will be made to provide the earliest possible appointment for the convenience of the patient. If you are unable to keep your appointment, you must cancel 24 hours prior to an appointment to avoid a fee. Please make sure you bring your insurance card(s) and photo ID to all appointments.
