Effective December, 21, 2022, Capital Health assumed operations from St. Francis Medical Center and renamed the location Capital Health - East Trenton. This site, located in the East Ward of Trenton, includes a satellite emergency department (SED) which is open 24/7 to the community. Also on the campus are an outpatient medicine clinic, the Capital Health School of Nursing and School of Radiologic Technology, and the C.A.R.E.S Child Wellness Program.

To request your medical records from services you received at St. Francis Medical Center prior to December 21, 2022, the following options are available:
- Call 609-394-4460 to request your medical records.
- Print a records request form and fax it to 833-280-8160 or email it to [email protected].
- Forms can also be obtained and submitted at the Hamilton Avenue lobby entrance at Capital Health – East Trenton.
- Visit one of our hospital locations between 8 a.m. and 4:30 p.m. to complete a medical records request form, or fax the form (available here) to one of the following:
