Capital Health Institute for Urban Care works to advance the health and well-being of Trenton residents by growing and aligning Capital Health’s services with a robust network of community partners in the City of Trenton to improve medical care while overcoming the complex social determinants of health.
Public health challenges are part of every city across the country, often with challenges to pathways for making improvements. At Capital Health Institute for Urban Care, we have been working to identify new ways of tackling Trenton’s critical health, education, economic, and environmental challenges for nearly a decade.
Through collaborations with the city’s health care providers, local government, and faith-based organizations, schools, and food banks, the Institute aims to deepen Capital Health’s connections to Trenton and make meaningful and lasting improvements for its residents.

The Institute will coalesce, grow and align the services of Capital Health with those of the community and other partners to advance the health and well-being of the residents of Trenton.
