Capital Health has launched a new program that brings hospital-level care to patients in the comfort of their homes. Aimed at managing capacity at Capital Health hospitals by allowing appropriate patients to receive care at their homes, Capital Health’s new Hospital at Home Program delivers personalized care through daily virtual visits with a doctor, in-person nurse check-ins and services like physical therapy, lab work or nutrition support, all arranged by a multidisciplinary team.
