Capital Health offers a comprehensive outpatient diabetes education program that empowers patients to take control in the successful management of their disease. A physician referral is required for participation. This program is recognized by the American Diabetes Association and includes:
- Group sessions taught by a multidisciplinary staff
- Individual appointments are available based on provider’s recommendation
- Topics include Diabetes Overview, Acute/Chronic Complications, Medications, Meal Planning, Physical Activity, Stress Management, and Goal Setting
The program is offered in two-part sessions on Mondays from 9 a.m. to 2:30 p.m. during consecutive weeks.
