Capital Health’s Assisted Living Program (ALP) is a unique and vital service offered to seniors who reside in publicly subsidized housing in Mercer County. Licensed by the State of New Jersey, our program provides a full array of health care and supportive services that allow seniors to remain at home. Our staff is dedicated to fostering independence while honoring the values of respect, dignity and independence.

Services

ALP participants receive services that are person-centered and tailored to meet their unique circumstances and needs. These services may include:

  • Comprehensive assessment conducted by a registered nurse
  • Nursing services available on-call 24/7
  • Assistance with personal care including bathing, dressing and grooming
  • Medication administration
  • Assistance with transportation to community or medical appointments
  • Assistance with meals and nutrition
  • Supportive counseling regarding the challenges associated with aging
  • Referral to services such as:
    • Physical and occupational therapy
    • Skilled nursing and rehabilitative services
    • Podiatry
    • Medical equipment and supplies
    • End-of-life care
    • Acute care services
    • Vision care
    • Hearing care
    • Dental care
    • Housekeeping service

Our Team

The Assisted Living Program consists of highly trained, certified and licensed team members. Our team approach incorporates registered nurses, certified medication aides, and certified home health aides to provide care and service to seniors. Training for all team members is provided prior to and throughout their tenure with the ALP. Licensed and certified team members are required to maintain the highest standards set by Capital Health and the New Jersey Board of Nursing.

Eligibility

Participants must be residents of subsidized housing in Mercer County and must require assistance with at least three activities of daily living (walking, bathing, grooming, dressing, etc.). Once a referral has been made to the ALP, a registered nurse will conduct a comprehensive assessment to determine if the individual is eligible.

Funding

The costs of the ALP are funded through a Medicaid Waiver and administered by the Managed Long Term Service and Support (MLTSS) Program. This program is managed by the Managed Care Organizations (MCOs) in New Jersey. The major MCO providers include Horizon NJ Health Care, United Healthcare Community Plan, and Aetna. Each senior is most likely enrolled in one of these MCOs. The ALP bills the MCO directly and handles all of the paperwork related to those claims.
