Built on a foundation of team-based care, our doctors and nurse practitioners at Capital Health Primary Care - Brunswick Avenue work with support services such as social work, care management, and community behavioral health organizations to provide complete patient-centered care to the Trenton community. Capital Health Primary Care - Brunswick Avenue accepts most insurances and is Capital Health’s first Trenton-based practice to accept Medicaid Managed Care.
