Preparing For Your Visit

What to do when you get here.

At Capital Health, our goal is provide patients with excellent cancer care. In order to do so, we ask that patients do their part to simplify the processes we’ve established so that we can make your experience with us one that you’ll be happy with.

From our team of expert cancer doctors, multidisciplinary teams, and specialized staff to our leading-edge treatments and the highest levels of quality care, we are here to guide you during your cancer journey.

Because every appointment is different, your healthcare provider will tell you where you should go when you arrive. If you do not know where to go, ask your provider before your appointment to ensure a smooth check-in.

Our front welcome desk will be happy to direct you to the appropriate floor and office for your appointment.

We encourage you to prepare for your visit. We are happy to answer your questions and address any concerns you may have about your visit. 

FORMS

Also, be prepared to tell the nurse what allergies you may have, the medications you are currently taking, and information about your health history and family health history. Before your visit, inquire if there are any forms you can complete prior to your visit. Some forms may be found here.

PARKING

A dedicated Cancer Center patient parking lot is available immediately outside of the Cancer Center entrance. Complimentary valet parking at the main entrance exists as well! Click to learn more about our various parking options.

QUESTIONS

Prepare a list of questions about what you can expect after your first appointment, the next steps in your diagnosis, or questions about treatment at the Capital Health Cancer Center.

SCANS/TEST RESULTS

Your healthcare provider may request to see any available scans (CT, PET, or other imaging), pathology/biopsy slides, or doctors’ notes related to your diagnosis. Ask your healthcare provider whether they would like these sent to their office or whether you should bring a copy (CD, DVD for imaging, slides for biopsies) with you during your appointment. Your doctor may be part of the Capital Health network with access to our shared electronic medical records system.

VISITOR INFORMATION

You may wish to ask a family member or friend to come with you to your appointment to take notes. For more information on COVID-19, visitor restrictions, and vaccine updates, please refer here.

WHAT TO BRING

  • Referring physician name, address, telephone, and fax number
  • Medication list (drug name, dosage, and frequency)
  • Treatments and timeline, if any have taken place
  • Imaging and test results requested by your provider
  • Insurance information and card
  • Prescription card and pharmacy contact information
  • Pre-certification and/or a referral from a primary care physician if required by your insurance company
  • Pen and paper to take notes
  • List of questions to ask your healthcare team
  • Calendar to keep track of appointments

Patient Responsibility

At the Capital Health Cancer Center, you are an integral part of the planning and treatment for your cancer journey. That’s why we’re relying on you to do your part to give us the best chance at success and why you have the responsibility to:

  • Provide to your practitioner, to the best of your knowledge, accurate and complete information concerning present medical conditions, past illnesses, hospitalizations, medications, advance directives, and other matters relating to your health.
  • Report any unexpected changes in your medical condition to your practitioner.
  • Advise your practitioner whether you comprehend a contemplated course of action and what is expected of you.
  • Ask questions.
  • Follow the treatment plan recommended by your practitioner.
  • Accept the outcome of your decision if you refuse treatment or do not follow your practitioner’s instructions.
  • Keep scheduled appointments, but if you are unable to do so, notify your practitioner or the healthcare facility.
  • Assure that the financial obligations of your health care are fulfilled as promptly as possible.
