CANCELLATION POLICY

If you are unable to keep your appointment, you must cancel at least 24 hours prior to an appointment. We have a service who calls to remind you of your appointment two days in advance. Failure to keep an appointment (or to cancel) will result in a NO SHOW fee.

PAYMENT POLICY

Please make sure you bring ID and insurance cards to all appointments. Your co-payment must be received at the time of your appointment.
