The results are enhanced quality of life and well-being.
Through personalized exercise, nutritional counseling and other risk factor management approaches, individuals regain control of their lifestyle behaviors. Most programs begin with a consultation and interview prior to entering the program. The individual assessment should include a health history and review of risk factors, needs assessment and a plan of care developed and implemented toward a healthier lifestyle.

Outpatient Cardiac Rehabilitation
Those recovering from a recent heart-related illness or treatment procedure should begin with a personalized program consisting of telemetry monitored and supervised exercise, plus lifestyle modification. The program assists individuals to understand, cope and live successfully with heart disease. Individuals enter the program with a physician's referral after stabilization and hospital discharge. The individual's physician participates in the plan of care and receives reports on their progress. Each participant in this phase of the program should have an individualized consultation with a registered dietitian.

Cardiac Fitness Program
A cardiac fitness program serves individuals with stable heart disease, those who are at a health risk or those who want to enhance their physical and psychological well-being. With a physician's clearance, participants begin a progressive exercise routine. They learn the principles necessary to obtain and maintain an exercise plan and to make important lifestyle changes. This is a follow-up to most cardiac rehabilitation programs.
