What We Do

While beneficiaries seeing health care providers in Capital Health’s Accountable Care Organization (ACO) have the freedom to choose doctors inside or outside of the ACO, Capital Health’s ACO provides coordinated primary and specialty medical care.

Our ACO has been developed based upon the concepts of the Institute for Healthcare Improvement’s Triple Aim and has the specific goal of assuring that the highest quality patient experience is provided in the most efficient and cost effective manner. To assure sufficient patient access, Capital Health’s ACO offers services at participating physicians’ offices, its two hospital locations in Trenton (Capital Health Regional Medical Center) and Hopewell Township (Capital Health Medical Center – Hopewell), and outpatient services at Capital Health – Hamilton.

To support our participating primary care physicians, our ACO also provides specialty services that include participating general surgeons, clinical cardiologists, endocrinologists, sleep medicine specialists, pulmonologists, vascular surgeons, gastroenterologists, obstetricians and maternal fetal medicine specialists, neurologists, and neurosurgeons. Our ACO also uses participating hospitalists to provide efficient hospital care and continuity during transitions.

As a founding member of the Trenton Health Team, Capital Health has been working with a Federally Qualified Health Center (FQHC) partner, Henry J. Austin Health Center, to enhance access and services to beneficiaries who live in Trenton by linking our Family Health Center (located on 433 Bellevue Avenue in Trenton, NJ) as an access point for the FQHC. Henry J. Austin is a member of our ACO and is critical in assisting us in meeting our Triple Aim goals in Trenton.
