Creating a Scholarship
One popular way to support students in the Capital Health School of Nursing is through scholarship assistance. While gifts in any amount may be directed to School of Nursing scholarships, you may prefer to create a specially named fund.

Scholarships may be designated for students within a particular locale or for those with a specific grade point average. The Foundation Office can work with you to see that your preferences are met, but fewer restrictions allow the School of Nursing to respond with greater flexibility to the needs of our students as they arise in any given year.

Credit Card
Credit card gifts have become a quick and easy way to give. Capital Health Foundation accepts all major credit cards (VISA, MasterCard, Discover and American Express.) If you would like to make a gift with your credit card, please call the Foundation Office 609-394-4121.

Matching Gifts
You can double or triple the impact of your gift by applying for a matching gift. Thousands of companies sponsor matching gift programs to health related charities, like the Capital Health Foundation. Many companies will also match the contributions of their retired employees and spouses of employees.

Please contact your Human Resources office to find out if your company has a matching gift program. If your company participates, please request the appropriate form and return it to us with your contribution.

Memorial or Tribute Gifts
You may choose to honor or memorialize a friend or loved one by donating to Capital Health Foundation. Your gift enables the health system to provide the most comprehensive medical care possible while celebrating the life or accomplishments of others.

Your request for a Memorial or Tribute Gifts should include:
  • your full name, address, and phone number
  • an indication as to whether the gift is in "Memory of" or in "Honor of"
  • the name of the person being memorialized or honored
  • the name and address of the person(s) to whom the acknowledgment should be sent
An acknowledgment of your gift will be sent to the person (or family of the person) being honored or remembered. The amount of the gift will not be disclosed.

Information for Funeral Directors
Capital Health is committed to improving the health of the community we serve. Families of the recently deceased can request that contributions be made in memory of their loved one.

If someone requests that gifts be made to Capital Health, please follow the following guidelines to insure proper receipt and acknowledgement of these gifts.

  • Please provide the Foundation Office with the name and address of the family member to whom acknowledgements should be directed.

Stock

Giving appreciated securities (stock) is a convenient way to support Capital Health while gaining tax advantages. You will receive an income tax deduction for the full market value of the stock on the date of the gift, provided you have owned the securities for more than 12 months. Most donors avoid liability for capital gains tax otherwise due on the sale of securities.

When the security (stock) is held by your broker or commercial banker:
Notify your broker that your want to give securities to the Capital Health Foundation and send a letter to the broker and the Capital Health Foundation. Include the amount of shares, the name(s) of the stock(s), and state your intention for the gift. In the letter, direct your broker to transfer your gift into Capital Health’s account. If the broker has any questions, he/she can call the Foundation Office 609-394-4121 for further instructions. To transfer stock to the Capital Health Foundation's account, your broker can use the following:

When the security (stock) is in your name and in your possession:

  • You must obtain the Stock/Bond Power from your broker or banker.
  • Sign the Stock/Bond Power and have your signature notarized. Sign exactly as your name appears on the stock certificate. If the security is jointly owned, both owners must sign the stock/bond power.
  • Mail the signed Stock/Bond Power and the signed Release Form in a separate envelope to the address below along with a copy of your letter of transmittal.
  • Mail unendorsed certificates by first class or registered mail with a letter of transmittal to the Capital Health Foundation.
  • If the certificates are already endorsed, send them by insured mail or deliver them in person to the Foundation Office.
Valuation of Your Stock
The gift value of securities is the mean of the high and low prices on the date that the stock shares are transferred by your broker and received in the Capital Health Foundation’s account or hand delivered or mailed (date of the later postmark if not the same) in negotiable form to the Capital Health Foundation Office.
Tree of Life

Located in the lobby of the 5th floor Inpatient Oncology Unit at Capital Health Medical Center – Hopewell, the Tree of Life symbolizes the hope, courage and gratitude of our patients and their families.

Gifts to the Tree of Life are used to improve medical care and services for patients and their loved ones.  Leaves are inscribed with the names of those who have courageously fought the battle of cancer, those who still fight on and those who have cared for our family and friends through this journey.

Baby Waves Wall

Located in the 3rd floor lobby area of Capital Health Medical Center – Hopewell, this commemorative and memorial baby wall is sure to delight all those who visit with it soothing colors and whimsical fish. These playfully animated fish can be inscribed with the name and birth date of your child. All donations benefit the Newborn Nursery and Neonatal Intensive Care Unit.
